Renaming sometimes fails, but works if you try again

Its probably a matter of adding some synchronized(...) bits, but sometimes when renaming large numbers of classes at once its possible to have duplicate target names in the output mappings.

When this hits the Mappings.apply which needs to populate a reverse lookup Recaf aborts the operation since you cant invert a 1-to-many mapping.
