Clean up FA documentation - noncode issue

All of our docs are way out of date, versions are wrong, etc etc.

All documentation (including how-tos) need a thorough review, cleanup, and updating.

Had a telecon w/ George Nassar from DMS the other day, generated a safe, stripped SQL load file w/ his help.

George is going to generate some notes for me after reviewing the extant docs and seeing what's missing/unclear/etc.

Haven't heard from George in 1+ months, but I finished off the first draft of installation instructions.
