Figure out why ETD submission that was supposed to be withheld did not include that in the provenance field. Check if It is also missing the author's email address. I think this has happened before, too. When I tested both a withheld and unrestricted ETD submission, both had those provenance fields.

I checked and am not able to submit an item without choosing one of the withheld options.

I found one other with this problem in the past few months and checked with the Graduate School about it.
