Describe the bug

This has been occurring over the last week or two.

I'm talking to Claude about issue A and will suddenly get a message talking about a different issue from my previous conversation, completely derailing the conversation and causing confusion. I need to disable and re-enable the Claude Code Chat extension and start a new chat to continue. (Starting a new chat without disabling and enabling the extension itself causes the issue to happen again)

This seems to happen when the previous conversation was extensive.

"Permission request timed out" errors seem to arrive hand-in-hand with the bug - not sure if it's a symptom of the bug or maybe a cause (e.g. a permission request times out and then "returns and is retried" in a different conversation)

VS Code (please complete the following information):

Windows 11, latest released (not insiders) version of VSCode, Claude Code and Claude Code Chat.

In the second example, the "previous issue" it started hallucinating about was related to the current issue being discussed, causing additional confusion. I continued further into the chat trying to correct Claude about what we were talking about and then noticed after a few more nonsensical responses that the bug was occurring again.
