feat(extensions): extensions can now include other extensions for grouping into one extension

Summary

BlockNote Extensions can now recursively include other extensions that they want to include.

This enables patterns like what I changed with the Collaboration extensions, which bundles multiple extensions into one "installation". This will enable bundling of more complex functionality into a single extension & encourage composition of extensions
