Motivation ---------- REST endpoints for scopes/collections have changed and we'd also like to support N1QL indices in scopes/collections. Modifications ------------- - Update REST endpoints for creating scopes/collections - Upgrade to couchbase-index-manager 2.0 and switch to running it from a global install (it uses a different Couchbase SDK version than fakeit) - Drop unsupported 4.x non-RBAC couchbase-index-manager invocation - Update build scripts to publish release 7.x versions. Results ------- Support for scopes/collections in 7.x targets.

Move to Enterprise 7.0.4 and Community/Enterprise 7.1.1
This provides a consistent implementation between couchbase-quickinit and couchbasefakeit. Because FakeIt has become a bit dated, it isn't compatible with modern Node or ARM64. We can have couchbase-quickinit remain more modern and install older tools (bloating size slightly) in this image for FakeIt support. Fixes #47
