feat[Orchestrator]: Allow skipping initial fetch defaults for orchestrator form widgets

Summary

• Add fetch:skipInitialValue to keep fields empty until user interaction
• Treat fetch:response:default: "" as a valid default
• Document the new option and behavior

Changes

• Update ActiveTextInput, ActiveDropdown, and ActiveMultiSelect defaulting logic
• Extend UiProps with fetch:skipInitialValue
